Who Should Be Called When a Retiree Dies?

EFH Benefits Plan

I. Life Insurance

A.    Call the Benefit Service Center at 1-844-4MY-WKDY (844-469-9539).

B.    The Benefit Service Center will open a claim on the Beneficiary's behalf.

C.    MetLife will contact the Beneficiary and request:

          1.     Death certificate

          2.     Social Security number, dates of birth and death of the deceased

           3.     Social Security number(s) of the beneficiary/beneficiaries with address/addresses and phone number(s)

D.    MetLife will send a checkbook to the beneficiary/beneficiaries, who may:

           1.     Leave the entire amount with MetLife and write checks on the account,

           or

           2.     Write one check for the entire amount and deposit it at the institution of their choice.

 

II. Retirement/Thrift Plan

A.    Call Fidelity Investments at 1-800-462-4015 for questions about survivor retirement benefits (for those who elected spousal benefits) and/or with questions concerning Thrift Plan beneficiaries.  The Retirement/Thrift Plans are administered by Fidelity.
